Support to the Health Sector Programme (GIZ-S2HSP) and Ministry of Social Development (MoSD) Province 1 for Implementing Healthcare Waste Management Project is a project of GIZ-S2HSP with its consultant partner Waste Services Pvt. Ltd. 

7 days Rapid Diagnostic Assessment of healthcare waste management system was conducted in 12 Provincial hospitals of Province 1 from 16th December, 2020 until May 2021.

The project’s objectives’ were to: 

  1. Conduct rapid diagnostic assessment in 12 Provincial hospitals for Province 1.
  2. Support on design of a healthcare waste (HCW) treatment center along with implementation plan including monitoring and supervision framework.

Support to Province 1 covered 12 Provincial hospitals of Province 1. Selected 12 Hospitals of Province 1 were:

  1.  Illam Hospital, Illam
  2. Inaruwa Hospital, Sunsari
  3. Sankhuwasabha Hospital, Sankhuwasabha
  4. Bhojpur Hospital, Bhojpur 
  5. Terhathum Hospital, Terhathum 
  6. Khotang Hospital, Khotang
  7. Udayapur Hospital, Udayapur
  8. Panchthar Hospital, Panchthar
  9. Okhaldhunga Hospital, Okhaldhunga
  10. Solukhumbu Hospital, Solukhumbu
  11. Taplejung Hospital, Taplejung
  12. Mechi Hospital, Jhapa (Only diagnostic assessment and implementation plan).

As one of the consultant partner, Waste Services Pvt. Ltd. had following responsibilities:

  • Conduct 7 days rapid diagnostic assessment of healthcare waste management system in selected 12 provincial hospitals of Province 1.
  • Prepare report on each hospital, based on the results of rapid diagnostic assessment of healthcare waste management system.
  • Provide a design for a treatment center including an environment-friendly technology in line with Government of Nepal applicable policies and guidelines, design drawings and BoQs should be provided of waste management center.
  • Develop healthcare waste management implementation plan of 12 hospitals including timeline with capacity measures and required process to establish healthcare waste management system.
  • Advice designated focal persons of the 11 hospitals (Mechi already covered) on planning and set up of HCWM. 
  • Co-ordination with Ministry of Social Development (MoSD), Provincial Health Directorate of Province 1 during implementation of this project.
  • Update GIZ-S2HSP and MoSD about progress and plan of assigned activities according to the TOR and in coordination with the responsible focal persons.

Activities carried out during the projects were:

  • Phase 1: Rapid support on assessment of HCWM measures.
  • Phase 2: Develop HCWM design and implementation plan.

    Output of the projects were:

    • Strengthened the healthcare waste management system which primarily includes healthcare waste management implementation plan including detail logistic list requiring for procurement of essential equipment and supplies.
    • Detail design of the waste treatment center consisting of an environment friendly technology i.e. autoclave and encapsulation, landfilling as a final waste disposal option method in line with the Government of Nepal applicable policies and guidelines was provided.
    • Respective hospital in the designing and planning of HCWM treatment center were supported. 
    • Detailed healthcare waste management implementation plan was available in 12 hospitals and detail engineering design with BoQ was also available in 11 hospitals (excl. Mechi Hospital).

    Two reports of each hospital containing two volumes were submitted.

    • Volume I: Detail diagnostic assessment report on HCWM which includes results and discussion containing waste quantification, waste transportation and final management, observational result, questionnaire survey result, exploration for sustainability of HCWM system, recommendations and action plan, implementation plan with different necessary logistics, training and capacity building needs among clinical and support staffs. 
    • Volume II: Engineering design and drawings, detail cost estimation, rate analysis prepared for each hospital which outlined the options for establishing or upgrading the essential elements of healthcare waste management system.

    As mentioned in Volume II report, the elements of healthcare waste management system includes:

    • A ward- based waste segregation system, using color coded, labelled bins.
    • Standardized procedures for the collection and safe transfer of waste, including infectious waste for treatment, sorting and disposal.
    • Dedicated treatment areas in which infectious waste is safely sterilized, e.g. using non-burning autoclave technology.
    • Waste storage areas where treated infectious waste and non-hazardous waste are held for final disposal.
